Tri‑Flexsi Home Health Care, Inc. is seeking a highly motivated and compassionate Registered Nurse to join our team as a contractor in the Greater Houston Area. Compensation ranges from $55.00/hr to $120.00/hr, paid semi‑monthly. We only serve clients in Houston and surrounding areas.
Job SummaryAs a Registered Nurse, you will plan, organize, and direct home‑care services, providing nursing care and community health education to patients and their families within their homes.

- Completes an initial assessment of patient and family to determine home‑care needs.
- Provides a complete physical assessment and history of current and previous illnesses.
- Regularly re‑evaluates patient nursing needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions; includes patient and family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health‑care instructions per assessment and plan of care.
- Identifies discharge‑planning needs and implements them prior to patient discharge.
- Acts as case manager when assigned, coordinating care for assigned caseload.
- Communicates with physician regarding patient needs and reports any changes; obtains/receives physician’s orders as required.
- Communicates with community health related persons to coordinate the care plan.
- Participates in on‑call duties as defined by the on‑call policy.
- Ensures arrangements for equipment and other necessary items and services are available.
- Instructs, supervises, and evaluates home‑health aide care.
- Registered Nurse with current licensure in the state.
- Graduate of an accredited National League for Nursing school of nursing.
- Current CPR certification.
- Licensed driver with a vehicle insured per state or organization requirements and in good working order.
- Minimum of 2 years experience, with at least one year in public‑health or home‑care nursing preferred.
- Self‑directed and able to work with minimal supervision.
- Excellent observation, problem‑solving, verbal and written communication skills; nursing skills per competency checklist.
- Ability to organize and prioritize workload independently.
- Responsible for supervising home‑health aides; management experience not required.
- Physical abilities: prolonged walking or standing; able to lift, position, or transfer patients; lift supplies and equipment; reach, stoop, bend, kneel, crouch; adequate visual acuity and hearing for required nursing skills.
