• Deliver pharmaceutical care services through direct, innovative interactions with patients and healthcare providers.
• Conduct both face‑to‑face and virtual consultations to manage chronic disease states with cutting‑edge medication management.
• Utilize advanced techniques to obtain and analyze comprehensive medication histories for optimized patient care.
• Reconcile patient medications during ambulatory care visits, ensuring safety and efficacy.
• Engage in physical assessments and patient triaging as necessary, employing the latest diagnostic tools.

• Design, implement, and assess therapeutic plans, utilizing state‑of‑the‑art, evidence‑based practices.

  D or equivalent experience from an accredited pharmacy school.
• Completion of an ASHP‑accredited PGY‑1 Pharmacy Residency.
• Active license from the Texas Board of Pharmacy.
• Certification in Basic Life Support, First Aid, and CPR.
• Desirable:
  Board Certification as a Pharmacotherapy Specialist or Ambulatory Care Pharmacist.
• At least 2 years of community pharmacy and medication adherence counseling experience preferred.
• Experience in specialty areas such as HIV/AIDS, geriatrics, or pediatrics is advantageous.
• Proficiency in computer applications, including Windows, Microsoft Word, and Excel.
• Ability to manage multiple priorities and deadlines effectively.
• Excellent communication and customer service skills, with a strong sense of professional discretion.
• Desirable:
  In‑depth knowledge of advanced pharmacy practices and evidence‑based medicine.
