- Provide skilled home health occupational therapy interventions under the supervision of a licensed Occupational Therapist (OT)
- Implement and carry out individualized Plans of Care (POC) developed by the supervising OT
- Provide treatment for orthopedic, neurological, cognitive, and post-surgical conditions
- Address ADLs and IADLs, including bathing, dressing, grooming, toileting, feeding, and home management tasks
- Facilitate improvements in upper extremity strength, coordination, ROM, balance, and functional mobility
- Implement fall prevention strategies and home safety interventions
- Train patients and caregivers on adaptive equipment, assistive devices, and durable medical equipment (DME)
- Recommend environmental modifications to improve safety and independence
- Complete accurate and timely documentation in the EMR
- Communicate effectively with the supervising OT, physicians, and interdisciplinary team members
- Follow all home health regulations, infection control standards, and safety guidelines
- Participate in QAPI and agency performance improvement initiatives

- Associate Degree from an accredited Occupational Therapy Assistant program
- Active Texas Certified Occupational Therapy Assistant (COTA) license
- Home health occupational therapy experience preferred (1+ year preferred)
- Knowledge of Medicare home health documentation standards preferred
- Current CPR certification
- Strong communication, clinical judgment, and time-management skills
- Valid driver’s license, reliable transportation, and current auto insurance
- Ability to travel throughout Dallas and the surrounding DFW areas
