The Houston VISN 16 Assist Team is a multidisciplinary group formed to support all community care facilities within VISN 16. The team's primary focus is on care coordination, including consult management, appointment scheduling, record retrieval, and consult closure, in alignment with the Key Performance Indicators established by the Office of Integrated Veteran Care. Flexibility is essential for the VISN 16 Assist Team, as we may be called upon by the VISN at any time to relocate to any station within VISN 16.
