Rehab Without Walls, an affiliate of Bright Spring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside institutional settings and into their natural home and community environment through a continuum of care. • Conduct comprehensive evaluations and assess social, emotional, and support needs of patients and families, setting measurable objectives with the rehabilitation team.
• Identify and assist patients and families in utilizing community resources to achieve goals.
• Provide direct social work intervention, instruction, and counseling to cope with social and emotional responses.
• Reassess the effectiveness of treatment plans and modify goals as needed.
• Communicate patient needs and progress to the treatment team, physicians, patients, families, and others.
• Consult with medical providers regarding evaluation, treatment, progress, and discharge.
• Document and report changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager, or DCM, attending physician, and other team members.
• Maintain and submit evaluation, treatment, and billing documentation per company, facility, funding source, and regulatory requirements.
• Adhere to the company's code of conduct, policies, HIPAA, and compliance programs along with professional practice standards and applicable laws.
• Perform other duties as assigned.

• Current and unrestricted Licensed Clinical Social Worker (LCSW) licensure in the state(s) of practice.
• Master’s degree in Social Work from an accredited institution with LCSW licensure.
• Minimum of one year of experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or other acute diagnoses.
• Current BLS (Basic Life Support), First Aid, and CPR certifications.
• Effective interpersonal, verbal, and written communication skills.
• Ability to work independently and as part of a team.
• Strong organizational skills and attention to detail.
