Licensed Clinical Social Worker – Rehab Without Walls® Neuro Rehabilitation

Houston, TX | Salary: $78,000 - $95,000 |

Rehab Without Walls, an affiliate of Bright Spring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside institutional settings and into their natural home and community environment through a continuum of care. It is designed to help people with brain injury, spinal cord injury, or stroke/CVA regain functional capabilities by providing care in the home, community, workplace, or school, promoting greater health and improving functional skills and independence for patients.

- Conduct comprehensive evaluations and assess social, emotional, and support needs of patients and families, setting measurable objectives with the rehabilitation team.
- Identify and assist patients and families in utilizing community resources to achieve goals.
- Provide direct social work intervention, instruction, and counseling to cope with social and emotional responses.
- Reassess the effectiveness of treatment plans and modify goals as needed.
- Communicate patient needs and progress to the treatment team, physicians, patients, families, and others.
- Consult with medical providers regarding evaluation, treatment, progress, and discharge.
- Document and report changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager, or DCM, attending physician, and other team members.
- Maintain and submit evaluation, treatment, and billing documentation per company, facility, funding source, and regulatory requirements.
- Adhere to the company's code of conduct, policies, HIPAA, and compliance programs along with professional practice standards and applicable laws.
- Perform other duties as assigned.
- Current and unrestricted Licensed Clinical Social Worker (LCSW) licensure in the state(s) of practice.
- Master’s degree in Social Work from an accredited institution with LCSW licensure.
- Minimum of one year of experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or other acute diagnoses.
- Current BLS (Basic Life Support), First Aid, and CPR certifications.
- Effective interpersonal, verbal, and written communication skills.
- Ability to work independently and as part of a team.
- Strong organizational skills and attention to detail.
