This entry-level position introduces clinical pharmacy practice in the alternate site setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff and other professionals regarding appropriate medication use.

Job Description

The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use.

Job Responsibilities
  • Determines the suitability of individual patients for home care. Collects and organizes all patient-specific information needed to determine suitability. Assesses patients’ suitability for home care in accordance with Option Care policies and defines outcome goals.
  • Designs, recommends, monitors and evaluates patient-specific pharmacy care plans for pharmacotherapy of home care patients. Recommends modifications to prescribed therapy or monitoring parameters. Collects and organizes information to prevent, detect, and resolve medication-related problems. Identifies monitoring parameters for each patient. Completes nutritional assessment and makes recommendations.
  • Modifies care plans based on monitoring data and patient information. Applies pharmacokinetic principles and assesses patient laboratory values.
  • Provides medication-use education to Option Care staff, other professionals, home care patients and caregivers. Uses effective counseling techniques for patients and caregivers.
  • Ensures continuity of pharmaceutical care between home and other patient-care settings. Communicates pertinent patient information to and from home and other settings. Prepares and dispenses medications following policies and procedures. Calibrates equipment. Ensures medications are appropriately concentrated, stable, stored, and labeled. Adheres to safety and quality assurance practices. Maintains authorization to dispense and complies with state/federal laws, USP standards, and DEA regulations.

    Manages perpetual inventory for controlled substances and other medications subject to diversion.
  • Manages use, maintenance, and troubleshooting of medication administration equipment. Programs and advises on equipment use and maintenance. Participates in assessing, managing, and reporting medication-related incidents and equipment errors. Completes QAR documents per policy. Monitors inventory for discrepancies.
  • Provides concise, timely responses to drug information requests from health care providers and patients. Reviews literature to inform responses. Attends CEU opportunities to maintain competency and complies with state CEU requirements.
  • Manages investigational drug products according to regulations and Option Care policies. Uses pharmacy support personnel effectively. Prioritizes workload and organizes workflow to meet deadlines. Ensures accuracy of work by support personnel.
  • Complies with accreditation, legal, regulatory, and safety requirements in all activities.
  • Maintains confidentiality of patient and proprietary information in accordance with legal and ethical guidelines.
