Job SummaryThe Inpatient Social Worker Care Manager I (SWCM I) facilitates the collaborative interdisciplinary process of case management, including assessment, planning, facilitation, care coordination, and evaluation appropriate to the scope of licensure in a hospital setting. The SWCM I assists patients and families in addressing social, financial, and psychological problems related to health conditions, and advocates for services to meet comprehensive needs with available resources to promote quality, cost-effective outcomes.
The SWCM I provides clinical social work and complex discharge planning assistance to high-risk patients and families. This role may involve eligibility determination for social programs and assistance in qualifying for community resources. The SWCM I offers crisis intervention and/or mental health assessment, coordinates multidisciplinary discharge plans, and participates in interdisciplinary team meetings to address psychosocial issues across the continuum of care.
The SWCM I participates in quality improvement activities and promotes a customer-friendly environment using Service
FIRST behaviors.
Degree:
Graduation from an accredited school of Social Work with a Master’s degree in Social Work.
- Licensed Master Social Worker (LMSW), Required
- Current licensure in the State of Texas
- Related Specialty Certification preferred
- Basic Life Support
- Work Experience:
Prior experience as Social Worker preferred - Communication
