Position OverviewUnder the supervision of the Cohort Manager, the Case Manager will play a dynamic role as a trusted advocate for participants, helping them overcome challenges and achieve their goals. This position will be responsible for assessing participants' needs, developing personalized care plans, connecting them with vital services, and providing ongoing support to ensure they have the resources and tools to thrive.
Duties & Responsibilities- Conducts intake assessments and screenings to understand the unique needs of each participant. Work to identify service levels and prioritize participant needs, ensuring a holistic approach to care and support
- Develop, implement, and monitor individualized care plans integrating social determinants of health frameworks in case management plans. Acts as a strong advocate for participants, helping them make informed decisions regarding their health, emotional well‑being, nutrition, and life skills
- Collaborate with community organizations and public health programs to connect clients with appropriate services
- Actively engages with participants to ensure they take full advantage of available services. Provides ongoing support and guidance to ensure participants remain connected to the care and resources needed
- Coordinates care across multiple departments, community organizations, and other stakeholders, ensuring all parties are aligned and sharing necessary information. Works to prevent gaps in care and make sure participants receive comprehensive support
- Maintain and report accurate and timely data for surveillance and program evaluation. Accurately documents all interactions, services provided, and follow‑up actions in participant records. Ensures care plans and overall records are maintained and updated in accordance with organizational policies and confidentiality guidelines
- Educate clients and families about disease prevention, treatment options, and health system navigation by participating in community outreach events to promote awareness of public health services

Education- Bachelor’s degree from an accredited college or university majoring in Social Work, Human Services, Public Health, or a related field.
- At least two (2) years of experience in case management, care coordination, social services, or a related field.
- Valid Texas Driver's License.
- Possess or ability to obtain a Community Health Worker certification within nine (9) months of hiring.
Skills and Abilities
- Proficient in the use of laptop, tablet, and cellphone along with Microsoft Office Suite software
- Ability to engage with individuals from diverse backgrounds and collaborate effectively with a variety of partners
- Ability to manage multiple cases and priorities simultaneously; adapt to changing situations and work in a fast‑paced, team‑oriented environment
