Job Title/Position: Physical Therapist (PT)
Pay: $70 per regular visit, $75 per evaluation/assessment and $100 per start of care.

Reports To: Director of Nursing

At Home Quality Care was founded in 1983 in Morris, Illinois. We provide comprehensive home health and non‑medical home care services throughout the region, with seven branches in Texas, Illinois and Indiana.

We are looking for a physical therapist to make visits in the Cypress, Texas area who has experience in a home health setting. This role offers flexibility and the freedom to schedule visits around your life.

• Provide physical therapy services to patients according to a written physician’s plan of care.
• Assess and evaluate therapeutic/rehabilitative/functional status, and participate in plan development.
• Assess muscle strength, mobility, gait, ROM and potential for rehab.
• Direct physical therapy treatment.
• Instruct patients and families/caregivers in the use and care of therapeutic appliances.
• Determine priority needs for physical therapy.
• Report to physician patient’s reaction to treatment or changes in condition.
• Initiate physical therapy programs and, when appropriate, train patients in use of prosthetic devices.
• Identify patient and family needs for other home health services and refer as necessary.
• Prepare and submit clinical and progress summaries based on goal attainment.
• Participate in discharge planning.
• Provide physical therapy consultation to families and caregivers when indicated.
• Provide in‑service education programs for nursing organization personnel as needed.
• Participate in peer consultation process.
• Supervise physical therapy assistants per organization policy and state regulations.
• When therapy is the only skilled service, instruct, supervise and evaluate home health aide care every two weeks.

• Degree from an accredited baccalaureate or master’s program in physical therapy.
• Licensed to practice as a physical therapist within the state, if applicable.
• Two years of appropriate experience as a physical therapist; community/home health experience preferred.
• Strong verbal and written communication and organizational skills.
• Current CPR certification.
• Licensed driver with an insured automobile in good working order.
