Pay RangeYour pay will be based on your skills and experience. The base range is $70.00/hr - $70.00/hr.
Job Title/Position: Physical Therapist (PT)
Pay: $70 per regular visit, $75 per evaluation/assessment and $100 per start of care.
Benefits: Mileage reimbursement, company paid cellular data, paid vacation, and 401k (with match) for both part‑ and full‑time; health, dental and vision benefits available for full‑time employees.

Job OverviewWe are looking for a physical therapist to make visits in the Cypress, Texas area who has experience in a home health setting. This role offers flexibility and the freedom to schedule visits around your life.
Core Responsibilities- Provide physical therapy services to patients according to a written physician’s plan of care.
- Assess and evaluate therapeutic/rehabilitative/functional status, and participate in plan development.
- Assess muscle strength, mobility, gait, ROM and potential for rehab.
- Direct physical therapy treatment.
- Instruct patients and families/caregivers in the use and care of therapeutic appliances.
- Determine priority needs for physical therapy.
- Report to physician patient’s reaction to treatment or changes in condition.
- Initiate physical therapy programs and, when appropriate, train patients in use of prosthetic devices.
- Identify patient and family needs for other home health services and refer as necessary.
- Prepare and submit clinical and progress summaries based on goal attainment.
- Participate in discharge planning.
- Provide physical therapy consultation to families and caregivers when indicated.
- Provide in‑service education programs for nursing organization personnel as needed.
- Participate in peer consultation process.
- Supervise physical therapy assistants per organization policy and state regulations.
- When therapy is the only skilled service, instruct, supervise and evaluate home health aide care every two weeks.
- Degree from an accredited baccalaureate or master’s program in physical therapy.
- Licensed to practice as a physical therapist within the state, if applicable.
- Two years of appropriate experience as a physical therapist; community/home health experience preferred.
- Strong verbal and written communication and organizational skills.
- Current CPR certification.
- Licensed driver with an insured automobile in good working order.
