Patient Care Coordinator (The Ultimate People Person!)

Location:

Houston, TX | Full-Time | $18–$23/hr

Love helping people and keeping things organized? You might be our next superstar! At Harmony at Home Senior Care, we’re looking for a fun, fast-moving, compassionate go-getter to coordinate care, support our caregivers, and make sure our clients always feel like family.

• Communicate directly with clients and caregivers to ensure quality care and satisfaction.
• Tackle scheduling changes, caregiver call-offs, and daily adjustments like a champ.
• Help interview, onboard, and support new caregivers with a smile.
• Assign training, track progress, and keep compliance documents in order.
• Keep our databases (Swyft Ops, GHL) clean, organized, and up to date.
• Conduct check-ins, satisfaction calls, and performance reviews.
• Pitch in with marketing events, referral partner outreach, and community engagement.
• Provide administrative support and step in for the Administrator when needed.

• Previous experience in home care, home health, hospice, or similar healthcare setting.
• Energetic, upbeat personality with strong communication skills.
• Incredibly organized and loves solving problems.
• Available full-time, in office (Monday–Friday, business hours standard).
