Summary The Field Memory Connect Wellness Coordinator (MCWC) plays a key role in bringing supportive services to residents across multiple partner senior living communities while strengthening engagement with the Senior Doc Primary Care and GUIDE Dementia Support Programs
.
As a field-based team member, the MCWC travels to assigned communities, building meaningful relationships with residents, families, and staff. This role serves as a vital bridge between the care team and the community—ensuring patients with memory-related conditions receive personalized support, consistent care coordination, and proactive communication.
Responsibilities
- Travel daily to assigned senior living communities to deliver on-site support.
- Support dementia care activities through the GUIDE program.
- Provide in-person assistance for telehealth provider visits.
- Conduct care coordination tasks, including care plan updates, patient enrollment, and follow-up.
- Serve as the primary point of contact for residents and families within assigned communities.
- Provide companionship, engagement, and emotional support to residents.
- Foster collaboration with community staff to ensure seamless care delivery.
- Participate in community and company events, representing Senior Doc with professionalism and compassion.
- Provide event, marketing, and administrative support for field-based initiatives.
- Perform additional field duties as assigned.
Requirements
- Minimum 18 years of age.
- High school diploma, GED, or equivalent required.
- Medical Assistant Diploma (preferred).
- Bilingual (preferred)
- Education or experience in patient care or case management.
- Strong interpersonal and communication skills with professional, compassionate demeanor.
- Ability to work independently in the field and manage multiple assigned communities.
- Reliable transportation, valid driver’s license, and willingness to travel daily.
- Ability to stand/walk for extended periods.
- Flexibility to attend/work community events outside the typical M–F, 8–5 schedule.
- Phlebotomy license (preferred).
