Core Responsibilities
  • Provide direct care to external clients living outside Brookdale Senior Housing Communities, promoting the highest level of functioning as directed by the BAH Agency Leader and/or under the supervision of a licensed nurse when applicable by state rule.
  • Assist in maintaining the client’s environment in a safe and neat manner and promote quality services within non‑skilled home care standards and applicable state home care agency regulations.
  • Work independently in the client’s home with supervision and support per state regulations.
  • Receive and communicate changes in the client’s care needs to the BAH Agency Leader as needed.
  • Provide personal care and/or concierge/homemaker/companion services per the individualized plan of care.
  • Document all pertinent information regarding care as assigned.
  • Assist clients with activities of daily living including bathing, dressing, grooming, toileting, transferring and traveling to activities and meals according to the client plan.
  • Prepare residents for meals, assist to and from dining areas, set up and feed meals.
  • Assist residents with toilet and incontinence care, performing checks at least every two hours and providing incontinent care as needed when caring for more than two hours in a row.
  • Keep the client’s home neat and clean.
  • Perform vital signs, blood pressure, weight and other trained procedures per the care plan.
  • Complete preventive and restorative care including PROM, AROM, ambulation, bathing, grooming and other ADL care per the plan.
  • Maintain knowledge of the home care agency’s policies and procedures relevant to the role.
  • Assist in turning, positioning and repositioning clients.
  • Communicate effectively and proactively with clients, families, visitors and the BAH Agency Leader and scheduler.
  • Participate actively in the Community Quality Assurance Program.
  • Maintain ongoing education and training to remain compliant with state and federal regulations.
  • Maintain client confidentiality.
  • Assist with homemaker companion tasks such as laundry, shopping, light housekeeping, meal preparation per the plan.
  • Follow infection control and safety policies and guidelines.
  • Arrive on time for assignments by ensuring means of travel to client homes and reliable transportation.
Qualifications

Education and Experience

High school diploma or General Education Degree (GED) is required. Proof of completing an approved Certified Nursing Assistant program is required. Six months of experience working independently with clients in their own homes providing healthcare/home care services is preferred.

Certifications, Licenses, and Other

Special Requirements

Successful completion of the state CNA course, maintenance of CNA certification, reliable transportation, a valid state identification card and a valid driver’s license (preferred).

  • Standing, walking, sitting, using hands and fingers to handle or feel, reaching with hands and arms, climbing or balancing, stooping, kneeling, crouching, or crawling.
  • Talk or hear, taste or smell.
  • Ability to lift up to 50 pounds.
  • Vision required.
  • Interaction with co‑workers, residents, or vendors.
  • Occasional weekend, evening, or night work if needed.
  • Possible exposure to communicable diseases, infections, blood‑borne pathogens, latex, drugs, chemicals, or biological hazards.
  • Possible injury from animal or equipment, burns, odors, or cuts.
  • Requires frequent travel.
