The Service Linkage Worker provides services within a broad range of focus areas to assist patients in accessing and adhering to care. Services provided may include HIV testing and counseling, patient assessments, assisting to reduce access and adherence barriers, follow-up to ensure referrals are completed, patient navigation assistance, and coordination with case managers and other clinic staff.
Minimum QualificationsEducation /
Work Experience:
- Bachelor's degree from an accredited college or university with a major in social or behavioral sciences (AND) One (1) Year paid work experience in providing client services to Persons Living with HIV/AIDS (PLWHA)
- (OR)
- High School Diploma or GED (AND) Five (5) Years paid work experience in providing client services to Persons Living with HIV/AIDS (PLWHA)
