Clinical Pharmacist | Level 1 Trauma Center
Location: On-site, Military Base, Fort Sam Houston, TX 78208
Position Type: Permanent, Full-Time
Schedule: Monday–Friday, 8:00 AM–5:00 PM CT
Compensation: $57.69–$67.30/hour + $5K–$10K Sign-On Bonus
What You’ll Do:- Collaborate with multidisciplinary teams to optimize medication therapy and therapeutic outcomes.
- Provide direct patient care, including medication assessments, therapy monitoring, and patient education.
- Participate in emergency response, including Level 1 trauma activations, code blues, and massive transfusion protocols.
- Review medication regimens for appropriate indications, dosing, interactions, and duplication of therapy.
- Document pharmacy interventions, recommendations, and care plans in the medical record.
- Support discharge planning and ensure continuity of care from a pharmacy perspective.
- Develop, implement, and monitor clinical protocols.
- Provide consultative expertise to healthcare providers to improve medication usage.
- Monitor high-risk prescribing, including polypharmacy and high-dose regimens.
- Promote cost-effective medication utilization within the clinic.
- Valid pharmacy license in Texas.
- Bachelor's degree in pharmacy.
- Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
- Successful completion of an ASHP-accredited post‑graduate year one (PGY1) residency training program and ASHP-accredited post‑graduate year two (PGY2) or an ASHP‑accredited post‑graduate year one (PGY1) residency training program and three (3) years of clinical practice experience with BPS board certification.
- Meets standards of an ASHP‑Qualified Residency Preceptor within one year of employment.
- Current American Heart Association BLS and ACLS certifications.
- Minimum of 1 year of clinical pharmacy experience within the past 3 years.
- Valid driver’s license.
- Ability to meet ASHP Residency Preceptor standards within one year of hire.
Full‑time, permanent, on‑site role at a military base (travel may be required). Requires standing, walking, sitting, and bending. Occasional light lifting up to 20 pounds.
