Pharmacist, Clinical Cardiology | Level 1 Trauma Center

Location:

On-site | Military Base | Permanent Position

Schedule:

Monday–Friday, 8:00 AM–5:00 PM CT

Compensation: $57.69–$67.30/hour + $5K–$10K Sign‑On Bonus

Our client is seeking a Clinical Cardiology Pharmacist to join their team at Fort Sam Houston. This is a full‑time, on‑site opportunity for a driven, credentialed pharmacist who thrives in a collaborative, outpatient environment and is committed to delivering evidence‑based pharmaceutical care.

What You’ll Do:
  • Collaborate with a multi‑disciplinary team to optimize drug therapy and promote wellness.
  • Provide direct patient care, including assessment, treatment, and medication management.
  • Conduct daily clinical evaluations for patients in assigned areas.
  • Document pharmacy plans, interventions, and medication reconciliation in medical records.
  • Review drug regimens for indications, dosing, allergies, interactions, and duplications.
  • Provide timely, accurate drug information to medical staff.
  • Respond to level 1 trauma, code blue, and massive transfusion emergencies.
  • Monitor pharmacokinetics, renal/hepatic dosing, antibiotic stewardship, and patient education.
  • Support discharge planning and transition of care from a pharmacy perspective.
  • Monitor polypharmacy, high‑dose therapies, and ensure cost‑effective drug utilization.
  • Communicate professionally with patients, families, and healthcare professionals.
  • Help create the new pharmacist residency program.
What You Bring:
  • Valid pharmacy license in Texas.
  • Bachelor's degree in pharmacy.
  • Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Successful completion of an ASHP‑accredited post‑graduated year one (PGY1) residency training program and ASHP‑accredited post‑graduated year two (PGY2) OR another ASHP‑accredited PGY1 residency training program and three (3) years of clinical practice experience with BPS board certification.
  • Meets standards of an ASHP‑Qualified Residency Preceptor within one year of employment.
  • At least one year of pharmacy experience within the last three years.
  • American Heart Association BLS and ACLS certifications.
  • Valid driver’s license and ability to meet physical demands (light lifting, standing, etc.).
