Clinical Pharmacy Specialist - Hematology/Oncology
MD Anderson Cancer Center is recruiting a Clinical Pharmacy Specialist to provide expert medication therapy management in the Hematology/Oncology service line. The role is focused on safe, effective, and cost-conscious care through collaborative practice, research, and education initiatives.
OverviewThe Clinical Pharmacy Specialist participates in inpatient rounds, outpatient clinic coverage, on‑call, and weekend coverage as assigned. The specialist collaborates with a multidisciplinary team to optimize pharmacotherapeutic outcomes, manages medication therapy of individual patients, provides drug information, and ensures accurate medication reconciliation. The position supports institutional priorities related to formulary management, REMS programs, and medication safety initiatives.
Responsibilities- Develop and maintain an active clinical pharmacy practice on assigned service(s) and actively participate in inpatient rounds and/or outpatient clinic coverage.
- Collaborate with the multidisciplinary healthcare team to optimize the pharmacotherapeutic aspects of patient care.
- Manage medication therapy of individual patients, develop, recommend, implement, monitor, and redesign drug therapy regimens within scope of practice and institutional policy.
- Document daily patient medication problems and emergency situations in the electronic health record (interventions, notes, handoffs).
- Provide timely follow‑up on clinical effects of medication regimens and adjust therapy as needed.
- Provide drug and clinical information to other healthcare professionals and develop pharmacotherapeutic plans for patients.
- Perform accurate medication reconciliation and update medication lists.
- Identify and intervene appropriately to resolve potential medication problems of all assigned patients.
- Participate in discharge planning for assigned inpatients and proactively assess patient financial status to address barriers.
- Participate in institutional initiatives to develop, validate, and implement research and patient care tools (ordersets, treatment plans, algorithms).

D. degree. - PGY1 Pharmacy Practice Residency and PGY2 Residency in Hematology/Oncology or equivalent experience as a clinical pharmacist in the area of specialization may substitute for the required residency on a one‑to‑one basis.
- Texas State Board of Pharmacy pharmacist license within 90 days of job entry.
- Board Certification in an appropriate specialty area (Board of Pharmacy) preferred.
- Experience as a clinical pharmacy specialist preferred.
