PRN Social Worker, Masters Licensed

Position Overview

PRN Social Worker, Masters Licensed (Baylor St. Luke's Medical Center – McNair Campus, Houston, TX). The role is part of the Acute Rehabilitation Unit (ARU) at a newly relaunched 30‑bed inpatient rehab center that supports patients recovering from serious conditions such as stroke, traumatic brain injury, spinal cord injury, complex neurological disorders, amputation, and orthopedic injury.

• Specialized inpatient rehab focused on restoring function and independence after major illness or injury.
• Operated by Lifepoint Rehabilitation, the largest manager of hospital-based acute rehab units in the U.S.
• 24/7 hospital operation ensuring continuous care and support.

• Assess and plan interventions to help patients cope with social, emotional, economic, and environmental problems.
• Interview and assess patients, families, caregivers, and legal representatives.
• Determine, prioritize, and arrange for needed internal and external services/interventions.
• Participate in case reviews and consult with the healthcare team to promote, monitor, and evaluate compliance with the patient’s treatment plan.
• Assist with discharge planning and processes, identifying appropriate resources and providing referrals.
• Supervise and/or train new staff, students, and interns.
• Provide individual, family, and group therapy.

• Bachelor’s degree required;
  Master’s preferred.
• Licensed as a Board Certified Social Worker or related field (e.g. counseling) in the state of Texas.
• Handle With Care certification within 30 days of hire.
• Strong critical thinking, decisive judgment, ability to work with minimal supervision, and ability to function in a stressful environment.
