Legacy Community Health is hiring immediately for Community Health Advocate through our partnership with the Ameri Corps Program. Ameri Corps is a Federal Program that connects Americans with Community Services in Health, Public Safety, etc.
The session begins March 2024. Must commit to completing the 1700-hour requirement by August 31, 2025. The bi-weekly (every two weeks) stipend is $700, less applicable taxes.

About the PositionReduce barriers to accessing primary and preventative healthcare services through connecting Legacy patients and community members to resources that address social determinants of health.
Key Responsibilities- Assist Legacy patients and community members in identifying and addressing barriers to receiving healthcare services.
- Provide health education and screenings with a goal of increasing health outcomes and access to care.
- Engage in health fairs, community meetings, and presentations to increase community reach.
- Assist in the assessment of community health needs through utilization of nationally recognized social determinants of health screening tools, including PRAPARE and the Health-Related Social Needs Screening Tool.
- Utilize a database of assistance organizations to connect individuals with identified social determinants of health barriers to available resources.
- Provide coordination and support to patients accessing community resources and services.
- Perform other duties as assigned.
In addition to the activities above, all Legacy Community Health Corps Members will:
- Represent Legacy’s Ameri Corps Program by wearing the Ameri Corps logo daily.
- In the event of a local, state, or federal disaster declaration, Ameri Corps members may be asked on a volunteer basis or be required by One Star to activate to assist in disaster response and recovery activities within their communities.
Nonessential Functions:
Legacy Community Health Corps Members may have opportunities to engage in the following activities:
- Work with community partners to identify service opportunities in alignment with the program design.
- Members may be asked to perform other position-related instructions within the scope of their normal service activities as requested by their site supervisor and/or Ameri Corps program staff.
- Provide translation and interpretation services as appropriate.
Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay. Our goal is to treat the entire patient while improving their overall wellness and quality of life.
Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to >50 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park.
Requirements- High School diploma or GED is required.
- No previous experience is required.
- Bilingual / Spanish fluency is preferred.
- U.S. Citizen or a Permanent Resident.
