As a Physical Therapist, you will evaluate residents, develop personalized treatment plans, and implement therapy services to restore mobility, relieve pain, and promote functional independence. You will also be responsible for documentation, interdisciplinary collaboration, and maintaining compliance with all applicable regulations.

• Evaluate patients’ physical conditions and functional limitations.
• Develop and implement individualized treatment plans using evidence‑based practices.
• Administer physical therapy interventions to improve strength, balance, and mobility.
• Monitor patient progress and adjust treatment plans as needed in coordination with the IDT.
• Educate residents, families, and caregivers on care techniques and exercises.
• Ensure accurate, timely documentation per facility and regulatory standards.
• Attend team meetings and collaborate with other healthcare professionals to ensure optimal care.
• Follow all safety protocols and regulatory guidelines.

• Active Texas Physical Therapist license.
• In good standing with all relevant licensing boards and agencies.
• Experience in Skilled Nursing Facilities (SNF) or long‑term care preferred.
• Familiarity with Net Health documentation system is a plus.
• Strong interpersonal skills and the ability to work with diverse patient populations.
