Monday-Friday (10AM-7PM) and Occasional Saturdays

As a Health Navigation Specialist, you will:

  • Forge groundbreaking connections with community members and Legacy patients.
  • Facilitate seamless access to medical care and enhance engagement in healthcare journeys.
  • Leverage Legacy Pharmacy partnerships to disrupt traditional medication adherence for HIV prevention and treatment.

Key Responsibilities
  • Implement dynamic health education programming in both individual and group settings utilizing innovative teaching methods.
  • Employ agile navigation techniques and motivational interviewing to guide patients in setting and achieving health goals.
  • Provide data-driven information, referrals, and assist with seamless service linkage.
  • Conduct state-of-the-art point-of-care testing for HIV and other health conditions.
  • Contribute to the development of breakthrough health education programs.
  • Collaborate within an interdisciplinary care team to enhance patient outcomes.
  • Perform brief, tech-enhanced assessments of patient needs, readiness for behavior change, and track progress over time.
  • Establish and maintain strategic referral processes and community networks.
  • Promote timely access to care through digital solutions and teach self-management skills.
  • Document patient services using electronic health records with precision and accuracy.
  • Manage patient schedules and achieve performance metrics using data analytics.
Minimum Qualifications
  • High School Diploma or equivalent;
    Bachelor’s Degree in a related field preferred.
  • 3 - 5 years of relevant experience in a healthcare or community services environment.
  • Minimum of 1 year of relevant experience if holding a Bachelor’s Degree.
  • Proficient in performing point-of-care testing with a tech-savvy approach.
  • Expertise in HIV care and prevention strategies.
  • Core competencies of HIV Prevention within 3 months of hire.
  • Phlebotomy training preferred.
  • Bilingual English/Spanish highly preferred to enhance communication and outreach.
  • Proficient in Microsoft Word, Excel, and other advanced computer programs.
