Main duties of the jobThe role
You will run nurse-led clinics including long-term condition management, immunisations, cervical cytology (if trained), and health promotion. You’ll be encouraged to work autonomously while being fully supported by the wider clinical team.
About usWe're a friendly, close-knit practice with a low turnover of staff, this vacancy has arisen due to one of our practice nurses retiring, there are regular team lunches and we have, as a whole a focus on well being, there are other Practice Nurses to share the workload and you will be continuously supported by the GP's. - Deliver nursing care in line with NHS and local policies, NICE guidance, and NMC standards.
- Undertake nurse-led clinics for:
- Long-term conditions (e.g. asthma, COPD, diabetes, hypertension)
- Immunisations and vaccinations
- Cervical cytology (if trained)
- Wound care and dressings
- Health promotion and lifestyle advice
- Carry out clinical assessments, monitoring, and follow-up care.
- Recognise, assess, and elevate clinical risk appropriately.
- Provide patient‑centred, compassionate care.
- Offer clear health education and self‑management support.
- Maintain accurate and timely clinical records on the clinical system.
- Work collaboratively with GPs, HCAs, pharmacists, and other services.
- Adhere to infection prevention and control policies.
- Participate in clinical audits and quality improvement activity.
- Support QOF and enhanced service delivery where appropriate.
- Maintain NMC registration and revalidation.
- Participate in appraisal, supervision, and continuing professional development.
- Attend practice meetings and training sessions.
- Work within scope of practice and seek support when required.
- Registered Nurse (NMC)
- Ability to work independently and as part of a team
- Good communication and organisational skills
- Commitment to patient‑centred NHS care
- Experience with clinical systems (e.g. Systm One / Emis)
- Experience working in primary care / GP practice
- Chronic disease management qualifications
- Cervical cytology qualification
- Immunisation and vaccination training
