• Collaborate seamlessly with the Lead Pharmacist and local services to deliver a top‑tier clinical pharmacy service across five inpatient wards, the HBPoS, the Hounslow Crisis Team and the MHCAS.
• Develop and implement medication management plans tailored to patient needs, ensuring optimal clinical outcomes.
• Engage in patient consultations and provide expert advice on medication use and general health issues.
• Stay abreast of new pharmaceutical approaches and contribute to continuous quality improvements in pharmacy services.
• Support and mentor junior pharmacists and pharmacy technicians, fostering a supportive team environment.
