Department/UnitSummary
Join our team in Inpatient Rehabilitation where you will provide compassionate care to patients alongside a dedicated team. You will play a crucial role in assessing patient needs, administering treatments, and ensuring patient comfort and safety. This position offers the opportunity to work in a collaborative environment focused on delivering high‑quality patient care.
How you’ll contributeAs supervised and delegated by a registered nurse, you will provide direct patient care in accordance with applicable scope and standards of practice. Reports to the Director or Manager of Department.
Essential Functions- Contribute to the assessment of patients by collecting data for analysis by an RN.
- Perform plan of care interventions, including specimen collection and wound care; may administer medication.
- Document patient care provided.
- Assist RN with development of plan of care.
- Assist patients with daily living activities including personal hygiene, elimination, nutrition, and ambulation.
- Monitor, document, and communicate patient status and responses to interventions with RN, physician, patient, and family.
- Advocate for the physical and emotional well‑being of the patient.
- Participate in performance improvement and continuing education.
- Provide quality patient care and work flexible shifts/units as needed.
- Maintain regular and reliable attendance.
- Perform other duties as assigned.

Applicants should have a current state RN license and possess a degree from an accredited nursing school. Additional requirements include:
- Education: Graduate of accredited LPN program.

Certifications:
Current LPN license in state of practice - Basic Life Support certification is required within 30 days of hire.
- One year of experience preferred.
- ASLS certification preferred.
