Department/Unit Summary

A Registered Nurse (RN) case management coordinates comprehensive care for patients, ensuring efficient and effective treatment throughout their hospital stay and facilitating a smooth transition upon discharge. By collaborating with multidisciplinary teams, they develop individualized care plans that address both medical and psychosocial needs.

Key Responsibilities:

• Patient Assessment: evaluate patients' medical conditions, support systems, and financial resources to determine appropriate care plans.
• Care Coordination: collaborate with physicians, therapists, and nursing staff to ensure patients receive necessary treatments and interventions.
• Discharge Planning: develop and implement discharge plans, arranging for follow‑up care, home health services, or rehabilitation as needed.
• Utilization Review: monitor the appropriateness of hospital admissions and continued stays, ensuring compliance with healthcare regulations and optimizing resource utilization.

Qualifications and requirements

• Applicants should have a current state RN license and possess an associate's degree from an accredited nursing school. Basic Life Support certification is required within 30 days of hire.
• One year of experience preferred
• ASLS certification preferred
