Medical Social Worker (MSW)
STGi is currently seeking a Medical Social Worker to provide services at our Community Based Outpatient Clinic.

The Medical Social Worker (MSW) will provide social work services under the direction of the CBOC Primary Care Physician, Psychiatrist, Nurse Practitioner, and the VA Medical Center (VAMC) personnel.
ESSENTIAL FUNCTIONS- Provide social work services based on physician’s orders to meet the needs (physical, mental, and emotional) of patients.
- Provide initial social work assessment for adult/geriatric Primary Care and/or Psychiatric patients.
- Conduct psychosocial assessments of Veterans and their support systems: evaluate health, living situation, family/caregiver supports, military experience, mental health, etc.
- Identify social, emotional, environmental, and financial needs; strengths, coping skills, barriers.
- Help patients cope with illness, injury, hospitalization, or medical treatment integrating the social, emotional, and physical aspects of care.
- Conduct psychosocial assessments of patients with medical conditions.
- Connect patients to resources (rehab, financial aid, housing, community support).
- Provide crisis intervention, grief counseling, or end‑of‑life support.
- Collaborate closely with physicians, nurses, and other health professionals.
- Understand medical terminology, treatment procedures, and the emotional impact of illness.
- Develop, implement, and monitor treatment/intervention plans in collaboration with Veterans, families, caregivers, and interdisciplinary treatment teams.
- Provide crisis intervention services when needed (suicide risk screening/intervention, emotional or environmental crisis, acute stressors).
- Offer therapeutic services: individual, family, group counseling or psychotherapy (depending on licensure and setting).
- Review progress of treatment(s) and provide appropriate feedback to a member of the multidisciplinary treatment team.
- Discharge planning: prepare Veterans for transitions from hospital or facility to home or another care setting; coordinate needed supports/placements.
- Case management/care coordination: ensure continuity of care, link to VA programs, community resources, benefit assistance, housing, financial assistance, etc.
- Education and advocacy: educate Veterans and families about VA benefits, community resources, rights; advocate for the needs of Veterans within and outside the VA system.
- High‑risk screening: identify Veterans at high risk for homelessness, suicide, frequent hospitalizations, inability to care for themselves.
- Provide mandatory reporting activities including Child Protective Services (CPS), Adult Protective Services (APS) and Elder Abuse.
- Perform cardiopulmonary resuscitation (CPR) and assist during respiratory and cardiac arrest procedures as applicable.
- Maintain documentation and case records consistent with VA and clinical standards.
- Ensure all patient information on access/wait time documentation is complete and accurate.
- Ensure all patient information on electronic encounter documentation is complete and accurate.
- Ensure that all required documentation and reports are completed in an accurate, timely fashion and complete fashion.
- Assist with the clinic’s compliance of all Federal, state, local, JCAHO, OSHA, Veteran Affairs, Contractor VA Program and Subcontractor safety and operational regulations, directives and standards.
- Maintain confidentiality of all information and support patients’ privacy, rights, and safety.
- Perform other work‑related duties as assigned.
- Prefer one (1) year of experience in working with mental health/psychiatric patients.
- Prefer 2‑3 years Medical Social Work experience.
- Current, full, active, and unrestricted license in Kentucky.
- Retain current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) from American Heart Association (AHA) or Military Training Network (MTN).
- Knowledge of and ability to apply professional social service principles, procedures, and techniques.
