• Provide in‑home speech therapy services for home health patients
• Perform evaluations and develop individualized treatment plans
• Address swallowing disorders, cognitive‑communication deficits, speech/language impairments
• Collaborate with nurses, therapists, and case managers
• Maintain accurate documentation in a Medicare home health setting

• Current Hawaii license as a Speech‑Language Pathologist (SLP)
• 1+ year clinical experience (home health experience a plus)
• Strong documentation and communication skills
• Ability to work independently in the field
• Valid driver’s license + reliable transportation
