Pharmacist, Healthcare

Position

The Pharmacist is responsible for the provision of patient care to meet Surgicare's identified patient outcomes, and to achieve the goals of the facility as directed by the administrative leadership. The Pharmacist performs duties according to scope of practice designated by the facility position description, role and responsibilities. This position is a per diem position and will ensure and provide outstanding pharmaceutical knowledge and services to our patients and their families.

As the Pharmacist, you will help to ensure that medication management is within standards of modern pharmaceutical care, quality audits are conducted and policies regarding medication management is up to date.

Supervision

Must be self-directed, willing to work with the Nursing/Clinical Manager and under the direction of the facility Administrator.

Minimum Qualifications:

* Current Hawai'i pharmacist license

* Current BLS certification

* MUST have experience working in outpatient setting or ambulatory surgery center.

* MUST have experience and knowledge of Joint Commission and CMS regulatory requirements.

Responsibilities:

* Provides pharmaceutical care and expertise

* Ensures appropriate, cost-effective drug utilizing while maintaining and enhancing patient care

* Audit charts for quality assurance

* Survey and oversee medication management

* Update facility medication management policies and procedures, ensuring they are up to date and within compliance with CMS and Joint Commission standards.
