Job Description:
Physical Therapist
Location:
Honolulu, HI 96817‑96825; in‑office and field‑based work.
Physical Therapist is to be a member of a multidisciplinary team to support and provide services for children from birth to age 3 with developmental delays or disabilities in their natural environment (home, daycare, etc.) serving the Honolulu area for Hawaii’s Early Intervention Services (EI/ EIS).
Job ResponsibilitiesPhysical therapy (PT) services include but is not limited to the following:
- Address the promotion of sensorimotor function through enhancement of musculoskeletal status, neurobehavioral organization perceptual and motor development, cardiopulmonary status, and effective environmental adaptation;
- Screening evaluation, and assessment of children to identify movement dysfunction;
- Obtaining, interpreting, and integrating information appropriate to program planning to prevent, alleviate, or compensate for movement dysfunction and related functional problems;
- Providing individual and group services or treatment to prevent, alleviate, or compensate for movement dysfunction and related functional problems;
- Conduct an Multidisciplinary Development Evaluation (MDE) to determine initial and on‑going Part C eligibility and/or to identify the child’s present level of functioning in each of the following developmental areas: cognition; physical (including vision and hearing); communication; social or emotional; and adaptive, and also include a review of pertinent records related to the child’s current health status and medical history;
- Function as a Primary Service Provider (PSP) to serve as the liaison between the family and other team members in order to support all outcomes and objectives in the child’s Individualized Family Support PLAN (IFSP) by utilizing coaching practices and receiving support from Consultants who represent the identified areas of need in order to support the child’s family strengthen their abilities in promoting the child’s learning and development within daily routines and activities;
- Function as a Consultant by utilizing coaching practices to support the PSP, parents, and other caregivers, to strengthen their abilities and build their capacity in promoting the child’s learning and development within daily routines and activities; and
- Participate in the IFSP meetings and other meetings (i.e., Family Support Team (FST) meeting, Joint Visit Planning (JVP) meeting, Transition meeting) regarding the child’s progress.
Physical Therapist must have a current, active State of Hawaii Licenses as Physical Therapist
Other Duties May Include- Participating in fundraising activities for UCPAH.
- Participating with UCPAH staff to ensure safety, sanitation and security of office assets of the organization.
- All other duties assigned.
