Base pay: $85,000.00/yr – $/yr (actual pay based on skills and experience).

Oahu (Island-wide travel required). Flexible/Per Visit or Full-Time. Setting:
Home Health.

• Deliver physical therapy services in accordance with the physician’s plan of care and state guidelines.
• Evaluate strength, mobility, range of motion (ROM), gait, and potential for rehabilitation.
• Create and direct individualized therapy plans.
• Educate patients, families, and caregivers in safe use of therapeutic equipment and exercises.
• Collaborate with the care team and participate in case conferences and discharge planning.
• Supervise Physical Therapy Assistants and, when appropriate, oversee Home Health Aide care.
• Document clinical progress and updates per agency policy and regulatory standards.

• Degree from an accredited physical therapy program (Bachelor’s or Master’s).
• Current Physical Therapy license in Hawaii (or eligible for Hawaii licensure).
• At least 2 years of experience as a Physical Therapist; home health experience preferred.
• Current CPR certification.
• Strong communication and organizational skills.
• Reliable transportation, valid driver’s license, and auto insurance (home visits required).
