Comfort Plus Home Healthcare is looking for a compassionate and motivated Physical Therapist (PT) to provide per diem home health visits in Miami, FL. Enjoy flexibility, autonomy, and the opportunity to make a real difference by helping patients recover and thrive, right in their own homes.

• Evaluate patient mobility and develop personalized care plans
• Provide skilled interventions to improve strength, balance, and functional independence
• Educate patients and caregivers on home exercise programs and fall prevention
• Supervise and guide PTAs as needed
• Monitor progress and update care plans accordingly
• Document visits accurately using Kinnser/Well Sky EMR

• Active Florida Physical Therapist license
• 1+ year of experience in physical therapy (home health preferred)
• Strong interpersonal and clinical skills
• Reliable transportation and willingness to travel
• Familiarity with or willingness to learn Kinnser/Well Sky EMR
• CPR certification

• Serve patients locally
• Supportive team and responsive clinical coordination
