Home Care Admissions Nurse

is looking to hire a full-time Home Care Admissions Nurse.

Position Summary:

The Home Care Admissions Nurse evaluates patients referred to home care services and determines the types of services to be provided. Conducts interviews of the patient and family to determine the appropriateness for services. Responsible for obtaining the initial and ongoing referral documentation from physicians, insurers and providers. Coordinates the development of home care plans specifying the frequency and duration of the services provided.

The Home Care Admissions nurse will complete admission, referral and insurance forms.

Requirements:

Education:

* Bachelors - Nursing (BSN) preferred.

Licensure/Certification :

* Current MA Registered Nurse (RN) license

* CPR Certified

Experience:

* One (1) to three (3) years home care experience

* Demonstrated computer / EMR skills
