Regalcare of Holyoke is a skilled nursing and rehabilitation center committed to delivering high-quality, resident-centered care and strong clinical outcomes. We are currently seeking an experienced, detail-oriented MDS Coordinator to join our leadership team in a full-time role.

• Coordinate and complete accurate, timely MDS assessments in compliance with state and federal regulations
• Manage the RAI process and support interdisciplinary care planning
• Ensure documentation supports clinical outcomes and reimbursement
• Collaborate with nursing, therapy, and administrative teams
• Monitor assessment schedules, audits, and quality measures
• Support surveys and quality improvement initiatives

• Active RN or LPN license in good standing
• Prior MDS experience in a long-term care setting preferred
• Strong knowledge of Medicare/Medicaid guidelines and RAI process
• Excellent organizational and time-management skills
• Ability to work independently and meet deadlines
