MDS Coordinator

A well-established skilled nursing and rehabilitation center in the Wakefield MA area is seeking an experienced and detail-oriented MDS Coordinator to join our team full-time. The ideal candidate will be a licensed nurse with strong clinical assessment and documentation skills who can ensure accurate and timely completion of MDS assessments and compliance with all state and federal regulations.

• Complete and coordinate MDS assessments in accordance with state and federal guidelines.
• Ensure accurate coding and documentation to support reimbursement and quality measures.
• Collaborate with interdisciplinary team members to develop and update care plans.
• Monitor resident progress and maintain regulatory compliance.
• Participate in care plan meetings and quality assurance programs.

• Current RN or LPN license in Massachusetts (required).
• Minimum 1 year of MDS experience in a skilled nursing facility preferred.
• Strong knowledge of RAI process, PDPM, and Medicare/Medicaid guidelines.
• Excellent attention to detail, communication, and organizational skills.
• Ability to work independently and as part of an interdisciplinary team.
