Full‑time / Regular (Monday‑Friday business hours).
STGi is currently seeking a Licensed Clinical Social Worker (LCSW) to provide services for a Community‑Based Outpatient Clinic in Holly Springs, MS.
Responsibilities- Provide social work services based on physician's orders to meet the physical, mental, and emotional needs of patients.
- Provide initial social work assessment for adult/geriatric Primary Care and/or Psychiatric patients.
- Participate in treatment completion, discharge planning, referrals, and placements of patients as applicable.
- Provide coverage for chemically dependent patients as applicable.
- Offer psychotherapeutic intervention of a specialized nature to patients and/or families with severe to complex psychosocial difficulties.
- Provide referrals for individual, group, marital, family, financial, or activity therapies.
- Carry out mandatory reporting activities including Child Protective Services (CPS), Adult Protective Services (APS), and Elder Abuse.
- Provide crisis intervention, supportive counseling, and problem‑solving interventions as applicable.
- Serve as a member of the multidisciplinary treatment team for assessment, diagnosis, and development of treatment strategies and goals.
- Review progress of treatments and provide appropriate feedback to the multidisciplinary treatment team.
- Act as primary liaison between families/friends of assigned patients, and between HNFS and the VAMC.
- Serve as a community liaison with VA and Community Mental Health Centers and other appropriate agencies.
- Act as a resource person to other CBOC staff regarding community services and programs.
- Provide group therapy in accordance with state licensure and regulations.
- Maintain universal precautions and infection control practices.
- Ensure smooth and timely flow of patients following VA access standards and triage protocol.
- Perform cardiopulmonary resuscitation (CPR) and assist during respiratory and cardiac arrest procedures as needed.
- Document all pertinent patient information in medical records and VISTA to demonstrate quality care delivery and promote continuity of care.
- Maintain the VISTA reminder tracking system current for each enrolled patient.
- Assist in scheduling patients for initial and follow‑up appointments in accordance with VA access standards and patient needs.
- Maintain patients informed about their plan of care.
- Provide health education, materials, and resources to patients and families.
- Assist the Clinic Manager in ensuring all patient information on access/wait‑time documentation is complete and accurate.
- Assist the Clinic Manager in ensuring all patient information on electronic encounter documentation is complete and accurate.
- Participate in the ongoing Performance Improvement Program among Contractor, Subcontractor(s), and the VAMC.
- Ensure all required documentation and reports are completed accurately and timely.
- Assist with the clinic’s compliance of all federal, state, local, JCAHO, OSHA, Veteran Affairs, Contractor VA Program and Subcontractor safety and operational regulations, directives and standards.
- Maintain confidentiality and support patients' privacy, rights, and safety.
- Perform other work‑related duties as assigned.
- Master's degree of Social Work from an accredited institution.
- Retention of a current state licensure in the state of practice.
- Retention of current CPR/BLS certification.
- Knowledge of and ability to apply professional social service principles, procedures, and techniques.
- Knowledge of crisis and family dynamics.
- Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergencies.
- Ability to set realistic, measurable education goals for patients, considering their and family needs, level of understanding, and available local resources.
- Ability to develop and provide individualized and group counseling, guidance, and problem‑solving assistance.
- Knowledge of basic customer service concepts related to age‑specific populations.
- Demonstrated effective verbal and written communication skills with proper telephone etiquette.
- Proficiency in medical terminology usage.
- Working knowledge of Microsoft Office and basic computer maneuverability.
- Minimum of one (1) year of experience working with mental health/psychiatric patients.
- Preferred:
Minimum of 2–3 years MSW/LCSW experience.
