As the LPN Home Care Coordinator, you’ll be a vital part of the interdisciplinary team, helping Life Circles participants receive the in-home care and support they need to thrive. You will coordinate a variety of home care services—including personal care, homemaking, respite, and community living support as well as durable medical equipment and supplies. You’ll ensure services are delivered efficiently, in line with established procedures, and always with participant well-being in mind.
Key Responsibilities- Conduct initial and periodic home care assessments in partnership with the IDT.
- Develop individualized care plans that support participants’ functional goals and home safety.
- Arrange and monitor services to assist with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
- Coordinate respite care to support caregivers.
- Ensure external providers and agencies clearly understand care plans and expectations.
- Utilize electronic health records and organizational tools to document and coordinate services.
- Partner with Physical and Occupational Therapists to ensure proper use of durable medical equipment (DME).
- Communicate regularly with the IDT regarding care plan progress, changes, and challenges.

Skills, Knowledge & Expertise- Current, active, and unencumbered Practical Nurse (LPN) license in the State of Michigan, required.
- Valid Michigan driver’s license and reliable transportation to conduct home visits.
- Minimum of 1 year of experience working with a frail or elderly population, required.
- 3 years of LPN experience in a geriatric setting, preferred.
- Ability to lift up to 35 pounds independently to support participants as needed.
- CPR certification through the American Heart Association (AHA), or willingness to obtain it through Life Circles.
