Schedule
: 40 hours per week | 8:00am – 4:30pm | Monday – Friday

Department: Life Circles PACE in Holland or Muskegon, MI

What You’ll Do in This Role

The Scheduler plays a vital role in keeping Life Circles PACE running smoothly by coordinating compassionate care for our participants. This position is responsible for scheduling Home Health Aides, building and maintaining contracted Home Health Agency schedules, and scheduling Life Circles van transportation appointments. With organization, attention to detail, and a heart for service, the Scheduler helps ensure our participants receive seamless, reliable care that supports their independence and quality of life.

Primary

Responsibilities
  • Schedule Life Circles Home Health Aides, Concierges, and Life Circles Van Transportation Services for participant appointments and approved care, ensuring timeliness and efficiency.
  • Build and manage schedules in the Electronic Health Record for Life Circles team members, contracted agencies, and transportation services, maintaining accurate service tracking.
  • Verify that all authorizations are in place for scheduled services.
  • Communicate schedules and changes to team members, participants, and care partners.
  • Monitor appointment availability and proactively escalate issues or conflicts as needed.
  • Establish positive, collaborative relationships with Life Circles team members, contracted agencies, and transportation staff.
  • Address participant questions, concerns, and scheduling conflicts professionally and empathetically.
  • Assist with on-call responsibilities as needed.

What You’ll Need
  • Healthcare scheduling, patient access, or medical office experience, required.
  • Minimum of two (2) years scheduling experience, required.
  • Minimum of (1) one year working with the frail and elderly, required.
  • Proficiency in Electronic Health Records, scheduling software, and database navigation, required.
  • Associate’s degree in healthcare, business, or related field, preferred.
  • Must possess current CPR certification by AHA or be willing and able to obtain as offered by Life Circles.
  • High school diploma or GED.
  • Must have reliable transportation and a valid drivers license.
