Job Details
- Must have current license and primary source verification within 30 days prior to start of assignment for all licensed professionals
- Must complete FACIS Questionnaire if any disciplinary actions
- Must complete HIPAA, Joint Commission - NPSG, OSHA Topics within a year of start and update yearly with minimum 80% score
- Must complete Ascension Core Competencies
- Must complete Ascension Biometric consent
- Must complete Ascension Non-Nursing Contingent Worker Pre-Hire Modules/Attestation Illinois
- Must complete Ascension Medical Imaging Contract Worker Onboarding Education Attestation
- Must provide forward facing headshot with white or blue background
- Must provide documented proof of two MMR vaccines or positive IGG Titer; no declinations accepted
- Must complete Fit Test annually
- Must provide documentation of Hepatitis B vaccination series completion or declination
- Must provide Influenza Vaccine during flu season or wear mask at all times
- Must provide positive IGRA or PPD results, X-ray within a year, and TB Questionnaire
- Must provide statement of good health or completed physical exam prior to first assignment, updated annually
- Must have valid Driver's License/State address on file
- Must complete 2 Step PPD OR IGRA within 30 days of start
- Must provide documented proof of two Varicella vaccines or serological evidence of immunity; no declinations accepted
- Must have valid Tdap vaccination within 10 years; no declinations accepted
- Must provide Ascension Profile (Resume)
- Must provide at least two professional references
- Must complete Ascension Criminal Background Check Results
- Must complete Ascension EPLS check
- Must complete Ascension Skills Checklist within one year of start and update annually
- Must complete 12 Panel Drug Screen within 30 days prior to start and update annually
- Must complete Ascension Education Verification within 90 days of start
- Must provide Ascension Confirmation of Assignment upon initial assignment and updates
- Must complete third-party employment verification for past seven years
