Position Overview

We are seeking an experienced Acute Care Pharmacist for a short-term acute care hospital setting. This role supports inpatient medication management, clinical dosing, compounding, and therapeutic monitoring. The pharmacist will work a rotating 7-on / 7-off block schedule with on-call responsibilities during scheduled work weeks.

Key Responsibilities
  • Verify and dispense medications for inpatient acute care patients.
  • Perform clinical dosing and therapeutic drug monitoring.
  • Manage anticoagulation therapy and warfarin dosing.
  • Perform renal, cardiac, pediatric, and pharmacokinetic dosing adjustments.
  • Prepare compounded and sterile medications using aseptic technique.
  • Utilize automated dispensing systems (Pyxis).
  • Collaborate with physicians, nursing staff, and interdisciplinary teams.
  • Maintain accurate documentation in Epic EMR.
  • Participate in after-hours on-call coverage during scheduled work weeks.
  • Comply with hospital safety, medication, and regulatory standards.
Schedule & Coverage
  • Shift: 4x10s | 07:00–17:00
  • Rotation: 7 days on / 7 days off
  • Weekends: Every other weekend (within rotation)
  • On-Call: Required during scheduled work week (negotiable)

Requirements
  • Licensure: Active State Pharmacist License (must be in hand at submission)
  • Experience: Minimum 1 year acute care / inpatient pharmacy experience
  • Certifications: IV Certification, RPh
  • EMR: Epic experience required
  • Skills Required:
  • Anticoagulation and Warfarin dosing
  • Cardiac, Renal, Pediatric, and Clinical dosing
  • Therapeutic drug monitoring
  • Sterile compounding and aseptic technique
