Job Title: Hospital Liaison
Department: Admissions / Marketing
Reports To: Administrator / Regional Director of Marketing or Admissions
FLSA Status: Exempt / Non-Exempt (per facility needs)
The Hospital Liaison serves as the primary point of contact between assigned hospitals, healthcare systems, and the long-term care facility. This role is responsible for building and maintaining referral relationships, facilitating timely and appropriate admissions, and ensuring a smooth transition of care from the acute hospital setting to post-acute long-term care or rehabilitation services. The Hospital Liaison plays a critical role in census development, patient advocacy, and regulatory compliance.
ResponsibilitiesHospital & Referral Relationship Management
- Develop and maintain strong working relationships with hospital discharge planners, case managers, social workers, physicians, and insurance representatives.
- Conduct regular hospital rounds and in-person visits as assigned.
- Represent the facility professionally at hospitals, meetings, and healthcare networking events.
- Serve as a knowledgeable resource regarding facility services, specialty programs, and payer contracts.
Admissions & Census Development
- Identify appropriate referrals and facilitate timely admission decisions.
- Coordinate with Admissions, Nursing, Therapy, and Business Office to ensure bed availability and clinical acceptance.
- Track referrals, admissions, denials, and conversion rates.
- Assist with census building strategies to meet occupancy goals.
Patient Transition & Care Coordination
- Communicate effectively with families regarding facility services, expectations, and admission processes.
- Ensure accurate transfer of medical records and required documentation.
- Support smooth transitions from hospital to long-term care or skilled nursing.
- Advocate for patient needs while aligning with facility capabilities and regulations.
Compliance & Documentation
- Ensure compliance with all federal, state, and local regulations governing admissions and marketing practices.
- Maintain HIPAA compliance at all times.
- Accurately document referral activity, admissions data, and follow-ups.
- Collaborate with facility leadership to ensure admissions meet regulatory and payer requirements.
Marketing & Community Outreach
- Promote the facility’s services, programs, and outcomes.
- Participate in community outreach initiatives and health system partnerships.
- Provide feedback to leadership on market trends, competitor activity, and referral patterns.
Education & Experience
- Bachelor’s degree in Healthcare Administration, Nursing, Social Work, Marketing, or related field preferred.
- Minimum of 2 years experience in healthcare admissions, hospital liaison, discharge planning, or post-acute care marketing preferred.
- Long-term care, skilled nursing, or rehabilitation experience strongly preferred.
Knowledge & Skills
- Strong understanding of hospital discharge processes and post-acute care continuum.
- Familiarity with Medicare, Medicaid, managed care, and insurance authorization processes.
- Excellent communication, negotiation, and relationship-building skills.
- Strong organizational skills with attention to detail.
- Ability to work independently and manage multiple priorities.
- Proficient in EMR systems, admissions software, and Microsoft Office.
- Ability to travel to hospitals and healthcare facilities as assigned.
- Ability to sit, stand, and walk for extended periods.
- Ability to lift up to 25 lbs occasionally.
- Flexible schedule may include evenings or weekends based on hospital needs.
- Maintain professional and ethical conduct at all times.
- Meet or exceed referral conversion and census goals.
- Maintain positive relationships with hospital partners.
- Adhere to all facility policies, procedures, and compliance standards.
