• Establish days and hours of practice.
• Control patient schedule and set appointment availability.
• Set fee schedule for examinations and services.
• Maintain 100% of exam/professional fee revenue.

• Advanced equipment/instrumentation.
• Administrative and/or opt‑tech support services (where state allows).

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school (if applying under ophthalmology).
• Valid state ophthalmology license.
• General liability certificate / professional liability insurance certificate.
