- Collaborate with RN-led team to provide patient-centered care.
- Establish authentic personalized relationships with patients and their support systems.
- Apply best scientific evidence and compassion in patient care.
- Maintain accountability for care provision in line with policies and procedures.
- High School Diploma or GED, required.
- Graduate of accredited Practical Nursing Program, required.
- One year of relevant experience, required.
- Current state LPN license, required.
- Computer skills, phone call etiquette, organizational skills.
