Job Summary

We are seeking an experienced Registered Nurse (RN) to provide patient-centered care in a Transitional Care Unit (TCU) supporting adult medical-surgical patients. The RN will be responsible for delivering high-quality nursing care, monitoring patient progress, and collaborating with an interdisciplinary healthcare team to ensure safe and effective patient outcomes.

Key Responsibilities
  • Provide comprehensive nursing care to adult patients in a Transitional Care Unit environment.
  • Assess patient conditions, develop and implement individualized care plans, and evaluate patient responses to treatment.
  • Administer medications and treatments in accordance with physician orders and facility protocols.
  • Monitor and manage specialized patient care equipment including wound vacs, chest tubes, tube feeding systems, NG/G-tubes, tracheostomy care devices, and PICC/PORT lines.
  • Accurately document patient care, progress notes, and clinical observations in the electronic medical record system.
  • Coordinate care with physicians, therapists, aides, and other healthcare professionals.
  • Provide patient and family education regarding treatment plans, medications, and discharge instructions.
  • Maintain adherence to infection control, safety policies, and professional nursing standards.
  • Support unit operations and assist in maintaining a safe and organized patient care environment.
Work Schedule
  • 13-week contract assignment
  • Approximately 36 hours per week after the first full week of orientation
  • Variable shifts and shift lengths depending on unit needs
  • Weekend and holiday coverage may be required
  • Flexibility is required as shift preference and hours are not guaranteed
Patient Care Environment
  • Transitional Care Unit with approximately 32 beds
  • Typical nurse-to-patient ratio: approximately 1:8 with aide support during the day and night
  • Nurses may be required to float to Medical-Surgical or Surgical Pediatric units as needed.
Equipment & Systems
  • Electronic Medical Record (EMR): EPIC
  • Additional equipment used may include Pyxis medication systems, wound vacuum devices, chest tubes, enteral feeding systems, tracheostomy equipment, and central line devices.
Minimum Requirements
  • Active Registered Nurse (RN) license in the State of Hawaii (must be obtained prior to the start of the assignment)
  • Minimum of 2 years of RN experience, preferably in Transitional Care Unit or Medical-Surgical settings
  • Basic Life Support (BLS/BCLS) certification required
  • ACLS may be required depending on agency or facility policy
  • Strong clinical assessment, critical thinking, and communication skills
Additional Requirements
  • Proof of influenza vaccination or an approved medical/religious exemption required prior to the start of the contract.
  • Orientation and department-specific training will be provided prior to beginning independent clinical duties.
