Experience in long-term care or skilled nursing preferred.

Assess and monitor resident conditions and vital signs.
Administer medications, treatments, and interventions safely.
Develop and follow individualized care plans.
Educate residents and families on health management and care plans.
Document care accurately and collaborate with interdisciplinary teams.
