Position Summary: Provides supportive psychosocial services to patients and families. Provides pre-death bereavement services, addressing such issues as family grief and loss, behavioral needs, and risk assessments for potential complicated grief. Provides services utilizing principles of palliative care and the hospice philosophy. Provides direct patient/family counseling and referrals to appropriate community resources as needed. Assists the patient and family in developing and enhancing optimal coping strategies.
Supports the hospice philosophy and team approach to provide, with other members of the team, holistic care to the patient and family.
