• Provide patient education on cardiovascular health, lifestyle modifications, medication management, and preventive measures to promote heart health and reduce cardiovascular risk.
• Maintain accurate and timely documentation of patient encounters, diagnostic findings, treatment plans, and follow-up recommendations in the electronic medical record system.
• Conduct comprehensive assessments, including history-taking, physical examinations, and diagnostic evaluations.
• Diagnose and treat a wide range of conditions.
• Develop and implement individualized treatment plans based on patient needs, preferences, and medical history.
• Collaborate closely with healthcare team to ensure coordinated and comprehensive patient care.
• Document patient encounters, treatment plans, and follow-up recommendations accurately and timely in the electronic health record system.
