MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. We also offer Elderplan/Home First: health plans for Medicare and dual-eligible individuals.

The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities.
Responsibilities- Provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care.
- Work in a community, long-term care environment, or hospital.
- Demonstrate specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review; may work independently or as part of a treatment team.
- Provide in-person visits or telehealth visits, as appropriate. Care will follow best practices, including history-taking, examination, care planning, treatment recommendation or implementation, communication, and documentation.
- Coordinate with the team physician and manager; perform face-to-face evaluations for hospice eligibility and manage hospice patients requiring exigent medical care.
- Contribute to training of the hospice team and support for the interdisciplinary team as needed.
- Help build a teaching curriculum on pediatric aspects of hospice care for hospice workers engaged in pediatric care through cross coverage or integration of young adolescents into adult hospice teams.
- Use an evidence-based approach to management and to the use of materials and policies/procedures in direct care and support to other clinicians.
- Engage in other activities related to quality improvement projects as appropriate.
- Master’s degree in nursing and graduation from an approved Nurse Practitioner program required.
- License/Certification: NYS Certified Nurse Practitioner (NP).
- Minimum of 1-year clinical experience as a NP provider preferred.
- Pediatric RN or NP experience strongly preferred.
- Prior experience as part of a specialist-level palliative care team or in a hospice environment preferred.
- CPR-BLS certification.
- Driver license and car required.
- Hospice & Palliative Care certification strongly encouraged by year 2 of hire.
- Proficiency in Microsoft Word and Excel.
- Ability to work flexible hours to meet the needs of the position.
