A Social Worker - Behavioral Health II is an integral member of the healthcare team, providing comprehensive support and services to patients and their families. The Clinical Social Worker - II is responsible for a range of duties, including independent psychotherapy, billing for services, conducting assessments, developing patient‑centered care plans, facilitating resources, monitoring progress and evaluation, advocating for patients, actively listening to their concerns, respecting individuality, and building trust.
This position is embedded within a primary care clinic located in Hillsborough, NC.
Responsibilities- Provide individual, group, and family psychotherapy using evidence‑based techniques.
- Accurately document and bill for services in compliance with insurance and regulatory requirements.
- Conduct comprehensive biopsychosocial assessments to evaluate patient needs and identify barriers.
- Collaborate with the healthcare team and patients to develop individualized care plans that integrate medical, psychological, and social aspects of care.
- Connect patients and families with community resources, mental health services, support groups, and other relevant programs.
- Provide crisis intervention and emotional support, and facilitate effective resolution plans.
- Continuously monitor and evaluate progress of patients’ care plans, adjust interventions as needed, and collaborate with the team for follow‑up.
- Advocate for patients’ needs within the healthcare system and community, ensuring access to appropriate treatment.
- Engage in active listening to understand patients’ experiences, concerns, and emotions, offering empathy, validation, and support.
- Respect the unique values, beliefs, and cultural backgrounds of each patient, tailoring interventions accordingly.
- Build trust‑based relationships with patients and families through empathy, confidentiality, and professionalism.
- Participate in meetings, problem‑solving, goal setting, quality improvement, and patient satisfaction initiatives, identifying and escalating opportunities.
- Master's Degree in Social Work
- Licensure or certification as a Clinical Social Worker (LCSW)
- Knowledge of mental health conditions, diagnostic criteria, and evidence‑based treatment modalities
- Strong assessment and diagnostic skills
- Excellent counseling and interpersonal skills
- Ability to collaborate effectively as part of an interdisciplinary team
- Empathy, compassion, and cultural sensitivity
- Strong organizational and time‑management skills
- Ability to maintain confidentiality and adhere to ethical guidelines
- Proficiency in billing and documentation procedures
