Job SummaryHillsboro Health and Rehabilitation Center is looking for a compassionate, organized, and dependable individual to support our Social Services and Admissions departments. The Building Coordinator plays a vital role in helping residents and families navigate care needs, transportation, and transitions, while working collaboratively with multiple departments. EssentialFunctions Social Services Support
- Assist with Admission, Quarterly, and Discharge Care Conferences
- Complete and document Social Services Admission, Quarterly, and Comprehensive Evaluations
- Support discharge planning with residents and families
- Arrange ancillary services as ordered or needed
- Provide or coordinate social services based on resident needs or concerns
- Assist with reviewing and following up on complaints or grievances
- Work under the direction of the Social Services Director
- Maintain compliance with all regulatory and facility policies
- Assist in coordinating new resident admissions and communicate with all departments
- Ensure resident rooms are prepared prior to arrival
- Obtain and process required admission paperwork
- Conduct facility tours and maintain a tour-ready environment
- Keep an up-to-date record of resident room assignments and bed availability
- Help orient residents and families to the facility and services
- Provide transportation and appointment support as needed
