IRC Admissions Coordinator – Atrium Health Wake Forest Baptist

Pay Range: $34.90 - $52.35 per hour

Education and Experience:

Bachelor’s degree in Nursing, PT, OT, ST, or another related field is required. Five years’ clinical experience in a hospital or rehabilitation setting preferred. Prior case management experience preferred. Knowledge of Inpatient Rehabilitation Facilities regulatory requirements preferred.

Licensure, Certification, and/or Registration: applicable state license required. CPR certification required. MOAB (Management of Aggressive Behavior) certification required but can be obtained after hire.

Essential Functions
  • Completes the pre‑admission assessment of candidates referred by acute care case managers, therapists, physicians, or external referral sources within twenty‑four hours Monday through Friday in accordance with CARF and CMS guidelines – Assessments include a review of the medical record, family interviews, and consultation with unit leadership and the rehabilitation physician to determine appropriateness of admission
  • Determines patient/family overall goal for rehabilitation, establishes a preliminary discharge plan and determines available insurance benefits for inpatient rehabilitation
  • Makes recommendations for inpatient rehabilitation or other appropriate post‑acute care settings and communicates recommendations to referral sources in a timely manner
  • Ensures appropriate pre‑admission documentation is complete
  • Facilitates daily admissions meeting with unit leadership and the rehabilitation physician to discuss potential admissions to the unit and any barriers that might exist including medical, clinical, financial, or social barriers
  • Ensures accurate collection of referral, denial, and admission data through maintenance of an Excel data file
  • Provides pre‑admission orientation to the unit including written materials such as the Inpatient Rehabilitation Center brochure and written disclosure statement
  • Seeks pre‑authorization from insurance companies and communicates availability of rehab benefits to prospective admissions
  • Works with financial counselors to assist uninsured clients with potential access to the rehab level of care
  • Collaborates with the physician and interdisciplinary team to develop and coordinate the patient plan of care
  • Participates in team conferences as needed
  • Keeps payer sources informed of patient progress and provides follow‑up information for utilization review and authorization of additional rehab days as needed
  • Improves the efficiency of the admission process, enhances communication, and improves patient outcomes
  • Provides accurate, thorough, and timely information regarding the clinical and financial status of new admissions to the team
  • Provides ongoing education to referral sources and staff regarding admission criteria and regulatory changes that affect the rehab level of care
  • Performs other duties as assigned

  • Basic knowledge of Excel and ability to conduct statistical analysis

Physical Requirements
