Overview

Licensed Practical Nurse (LPN) – Private Duty Nurse (PDN) at Avid Health  PDN-LPN is a member of the client care team providing skilled nursing services to clients in their personal residences, under the supervision of the Clinical Manager.

Responsibilities
  • Evaluates and treats clients with appropriate skill according to client’s needs under the supervision of an RN, Clinical Manager.
  • Assist with providing continuity of care and emotional needs of client and family.
  • Be resourceful in meeting client’s psychosocial needs.
  • Verbal demonstration of client rights and responsibilities.
  • Make decisions based on sound nursing judgments.
  • Recognize and cope with emergencies appropriately.
  • Ensure proper management of client services.
  • Demonstrate competent use of equipment.
  • Communicate information relevant to client care in accordance with agency policies.
  • Observe client’s general physical, emotional, and mental status, and report any changes to the agency RN/CM or Customer Care Coordinator.
  • Document all forms of communication with clients, families, and health plan providers promptly to promote continuity of care across interdepartmental functions.
  • Build relationships with clients to encourage compliance with care plans and client safety.
  • Maintain accurate clinical and shift documentation to update the system per agency guidelines.
  • Manage client/contract satisfaction with high standards of customer service, communication, and documentation.
  • Adhere to infection control policies and exposure control plan.
  • Monitor and maintain clinical reports, including Infection and Occurrence/Complaint Reports.
  • Promote safety and injury prevention in client and workplace settings.
  • Maintain and improve skills through staff development programs and in-service education requirements.
  • Participate in community outreach activities that promote the agency’s goals and objectives.
  • Maintain confidentiality in all professional areas and consider the needs of clients, families, other employees, physicians, visitors, and community members in a courteous and compassionate manner.
  • Ensure all HIPAA regulations are adhered to.
  • Follow Agency Policies and Procedures.
  • All other duties as assigned.
ADA Requirements
  • Routinely required to sit, stand, and walk on variable surfaces, with occasional climbing of ramps or stairs.
  • Routinely required to use hands for various tasks.
  • May be required to reach beyond mid-line of the body, reach overhead or chest level.
  • Vision requirements include close, distance, peripheral, depth perception and ability to adjust focus.
  • All employees must use universal safety precautions when performing tasks.
  • Ability to lift, push, pull and carry a minimum of 25 pounds.
  • Ability to stoop and bend up to 2.5 hours per 8-hour shift.
Qualifications
  • Education:

    Graduate of an accredited school of nursing.
  • Licensure/Certification:
    Licensed Practical Nurse (LPN) licensed by the North Carolina Board of Nursing (NCBON). Current, unencumbered LPN license in good standing. Must meet state and federal licensure/employment requirements.
  • Travel:
    Travel is required. Must have a valid driver’s license and satisfactory driving record.
  • Work Experience:

    Minimum of one year of home care experience. An additional year of nursing experience (preferably in a hospital) is required for conducting applicable training classes.
  • Other Hours of Work:
    Varies; full time is at least 30 hours per week (Saturday–Friday).
  • Skills and

    Additional Requirements:

    Understanding of branch office geographic/cultural requirements preferred; customer service oriented; able to liaise with patients, families, and other care providers; ability to use computerized systems; good verbal and written communication; ability to work well under pressure; bilingual is preferred.
